RESUMO

Os autores descrevem um caso raro de Pneumonia Intersticial Descamativa, num rapaz de 12 anos, internado com o diagnóstico de pneumonía adquirida na comunidade. A sintomatologia inicial foi de febre alta, tosse produtiva de expectoração mucopurulenta e cansaço fácil. A telerradiografia do tórax mostrava consolidação do campo pulmonar inferior direito. A persistência do quadro clínico e radiológico apesar da terapêutica antibiótica prescrita, motivou a realização de uma biópsia transtorácica que mostrou alterações compatíveis com Pneumonia Intersticial Linfóide, embora não pudesse ser excluído um processo linfoproliferativo, dada a pequena área de tecido estudada; por este motivo, o doente foi submetido a toracotomia exploradora a qual terminaria com lobectomia inferior dìreita e esvaziamento ganglionar, dada a aparência macroscópica daquele lobo pulmonar e gânglios linfáticos. Isto permitiu o diagnóstico de Pneumonia Intersticial Descamativa. Houve uma evolução favorável, com regressão completa do quadro clínico.

A etiopatogénese, diagnóstico e tratamento são revistos.

ABSTRACT

Authors describe a case of Desquamative Interstitial Pneumonia in a twelve years old boy, with a clinical picture of ligh fever, productive cough and fatigue. The chest X-ray demonstrated the existence of opacity of the right lower lobe. The persistence of the symtoms and radiological alterations, in spite of appropriate antibiotics, motivated the realisation of a thoracotomy that conclued in a righ lower lobectomy and gangliooar deplection, due to the macroscopic apearence af the lung lobe and the lymphadenapaties. The diagnosis of Desquamative Interstitial Pneumonia was made after histological examination of this lung lobe. The ethiopathogenesis, diagnosis and treatment are discussed.
