RESUMO

A transplantação pulmonar é hoje um método clínico bem estabelecido de tratamento cirúrgico de várias patologias pulmonares. Embora tecnicamente mais complexa que outros tipos de transplantação, nomeadamente a cardíaca, os resultados obtidos no imediato e médio prazo são excelentes, especialmente quando comparados com a história natural extremamente pobre das patologias de base. O principal obstáculo à transplantaçõ pulmonar é ainda a exdrema escassez de órgãos, uma vez que apenas cerca de 10% dos dadores multiorgão Odem ser dadores pulmonares. Por esta razão, a transplantação unipulmonar e, mais recentemente, a transplantação lobar são utilizadas, quando indicadas, de preferência à transplantaçã bilateral ou à transplantaçã de coraçã-pulmão. A incidência de complicações, nomeadamente as ligadas à rejeição e as infecciosas, é também mais elevada que noutros tipos de transplantação. Por esse motivo, o seguimento pós-operatório destes doentes tan que ser bastante mais apertado. É provável que o aparetimento de novos fármacos imunossupressores, mais selectivos, possa, num futuro breve, vir a alterar esta situação.

ABSTRACT

Lung transplantation is currently a well-established method of surgical treatment of several pulmonary diseases. Although it is technically more complex than other types of transplantation, namely heart transplantation, the immediate and mid-term results are excellent, especially when compared with the extremely poor natural history of the basal pathologies. The extreme paucity of organs remains the main obstacle to lung transplantation, as only about 10% of multiorgan donors are lung donors. For this reason, single lung transplantation and, more recently, lobar transplantation, are Performed, when indicated, in preference to bilateral lung transplantation or heart-lung transplantation. The incidence of complications, especially those related to rejection and infection, is also higher than in other types of transplantation. Hence, a much tighter follow-up is required. In the near future, the use of new, more selective immunossupressant drugs may, completely alter this situation.
