Anticorpos anti-VIH1 e VIH2 em doentes com Tuberculose. Experiência de um trabalho realizado ao longo de 3 anos nas regiões de Lisboa, Barreiro e Santarém

RESUMO

Pretendemos avaliar a taxa de anticorpos anti-VIH1 e anti-ViH2 em doentes com Tuberculose activa. A pesquisa de anticorpos foi realizada por ELISA e confirmada por Western Blot.

Incluímos 767 doentes (479 homens e 288 mulheres); 671 eram caucasianos, 76 negros e 20 de outras raças. Dezoito usavam drogas ilícitas por via endovenosa, 35 referiam ter feito transfusões de sangue fresco e 630 eram heterossexuais. Cento e cinquenta e três doentes tinham vivido em África por períodos variáveis de tempo; 541 doentes (70,5%) tinham Tuberculose pulmonar, 164 (21,4%) tinham Tuberculose extrapulmonar, 44 (5,7%) tinham simultaneamente Tuberculose pulmonar e extrapulmonar, sendo 12 formas miliares e 18 (2,3%) tinham Tuberculose primária.

Identificaram-se anticorpos anti-VIH em 14 doentes (1,8%): 10 anti­VIHl e 4 anti-VIH2; todos os doentes seropositivos eram heterossexuais e um toxicodependente; 5 doentes tinham exclusivamente Tuberculose pulmonar e 9 tiobam localizações extrapulmonares (entre estes, estavam todos os doentes com anticorpos anti-VIH2).

Encontrámos uma prevalência na nossa população de 1,8%, que foi mais elevada na raça negra (3,9%); a seropositividade foi mais frequente nos doentes com Tuberculose extrapulmonar. É aconselhável a pesquisa sistemática de anticorpos anti-VIH nos doentes com Tuberculose em actividade.

Setting

Patients with active tuberculosis.

Objective

To evaluate the rates of anti-HIV1 and anti-HIV2 antibodies in patients with active Tuberculosis.

Design

Anti-HIV antibodies, were searched by ELISA and confirmed by Western blot.

Results

We studied 767 patients (479 men and 288 women); 671 of them were caucasian, 76 black and 20 from other races. Eighteen of them were intravenous-drug users (IVDU), 35 had been transfused, 630 were heterosexual and in 121 we didn’t obtain data about sexual behaviour; 153 sojourned in Africa for different periods of time; 541 patients (70.5%) had pulmonary tuberculosis, 164 (21.4%) had extrapulmonary tuberculosis, 44 (5,7%) patients presented pulmonary and extrapulmonary tuberculosis and 18 (2.3%) had primary tuberculosis. Fourteen (1.8%) patients bad anti-HIV antibodies: 10 HIV1 and 4 HIV2; all seropositive were heterosexual, one being IVDU; 5 had pulmonary tuberculosis and 9 had extra-pulmonary tuberculosis (including all 4 cases with HIV2).

Conclusions

We found a rate in our population of 1.8% particularly in black patients (3.9%); serum positivity was more frequent in patients with extrapulmonary tuberculosis particularly in HIV2 patients. We advise for systematic screening.
